DCSS Guidelines for Activity History Comments

  1. Case Record Responsibilities
  2. All Title IV-D staff are accountable for proper activity history documentation.
  1. Activity history comments must be clearly written and easily understood by child support workers and court personnel.
  1. They are the electronic case record and may be subpoenaed for court and presented as evidence.  Staff may be called upon to explain unclear documentation at hearings.
  1. Activity History Requirements
  2. All activity history comments must:
  1. Meet safeguarding and confidentiality requirements.
  2. Be professional and business in nature.
  3. Be clear, concise, specific, and accurate.
  4. Be respectful, factual
  5. Be free from assumptions, judgments and personal opinions
  6. Be free from profane or vulgar language
  7. Be free from slang terminology and unapproved abbreviations.  See The PORT Glossary and Acronym List for approved acronyms.

  1. All case updates
  2. Conversations with parties of the case or others providing information (telephone, walk-in)
  3. Case actions
  4. Correspondences from any source (i.e. telephone calls, e-mails, faxes)
  5. Employer information
  6. Court updates
