DCSS Guidelines for Activity History Comments

- Case Record Responsibilities
- All Title IV-D staff are accountable for proper activity history documentation.
- Activity history comments must be clearly written and easily understood by child support workers and court personnel.
- They are the electronic case record and may be subpoenaed for court and presented as evidence. Staff may be called upon to explain unclear documentation at hearings.
- Activity History Requirements
- All activity history comments must:
- Meet safeguarding and confidentiality requirements.
- Be professional and business in nature.
- Be clear, concise, specific, and accurate.
- Be respectful, factual
- Be free from assumptions, judgments and personal opinions
- Be free from profane or vulgar language
- Be free from slang terminology and unapproved abbreviations. See The PORT Glossary and Acronym List for approved acronyms.
- What to document
- All case updates
- Conversations with parties of the case or others providing information (telephone, walk-in)
- Case actions
- Correspondences from any source (i.e. telephone calls, e-mails, faxes)
- Employer information
- Court updates
